About this procedure

The sworn statement on attached documents is a variation of the sworn statement in which, besides stating a fact under oath, you attach documents (the attachments) and declare that they are true, accurate and match what you're stating. It's very common in public and private tenders and contracting, when submitting case files, in banking procedures and with institutions that don't just want your word, but the supporting documents backed by that sworn word. What sets it apart from a simple sworn statement is precisely the tie between what you declare and what you attach: each document must be identified and referenced within the statement, so the whole set reads as a single body and not as a statement on one side and loose papers on the other. As with any sworn statement, the notary certifies that you appeared and swore; the truthfulness of the attachments is your responsibility. We draft the statement identifying and linking each attachment, making clear what is declared about each one, and we get it authenticated. If what you need is a simple statement without supporting documents, see Sworn statement; if you want the notary to acknowledge the signature on a private document that already exists, see Acknowledgment of a private document.
